In order to help us understand your company’s goals and needs, we ask that you fill out the form below prior to getting started with us. The form can be submitted online for your convenience. The signature page of the SaaS agreement may be emailed back to your sales rep, or faxed to us. The payment form you received via email should be faxed back to us at 866-211-7491.
Once we have received the completed documents, we will contact you to start the setup process for Lizzy.
